The following is a form of seeking urgent help from The Proof of Allah and the Lord of the Age, Imam Mahdi (PBUH).
Wherever you are, you may take ghusl, offer a two-rak’a prayer (beneath the sky), reciting Surah al-Fatihah and any other surah in each rak’a. Yet, it is preferable to read Surah al-Fath (48) in the first rak’a and Surah al-Nasr (110) in the second. You may then stand up, facing the kiblah direction under the sky and then say this supplication:
سَلامُ اللهِ الْکَامِلُ التَّامُّ الشَّامِلُ الْعَامُّ وَ صَلَوَاتُهُ الدَّائِمَةُ
May the perfect, complete, all-inclusive, and general salutations of Allah and His endless benedictions
وَ بَرَکَاتُهُ الْقَائِمَةُ التَّامَّةُ عَلَى حُجَّةِ اللهِ وَ وَلِیهِ فِی أَرْضِهِ وَ بِلادِهِ
and His everlasting, perfect blessings ,be upon the proof of Allah, His guardian on His earth and all lands
وَ خَلِیفَتِهِ عَلَى خَلْقِهِ وَ عِبَادِهِ
His deputy over all His creation and servants,
وَ سُلالَةِ النُّبُوَّةِ وَ بَقِیةِ الْعِتْرَةِ وَ الصَّفْوَةِ
the progeny of Prophethood, the remaining one of the lineage and the chosen one,
صَاحِبِ الزَّمَانِ وَ مُظْهِرِ الْإِیمَانِ وَ مُلَقِّنِ [مُعْلِنِ]أَحْکَامِ الْقُرْآنِ
the Lord of the Age, the manifestor of the (true) faith,the teacher of the laws of the Quran,
وَ مُطَهِّرِ الْأَرْضِ وَ نَاشِرِ الْعَدْلِ فِی الطُّولِ وَ الْعَرْضِ
the purifier of the earth, the spreader of justice all over the earth,
وَ الْحُجَّةِ الْقَائِمِ الْمَهْدِی الْإِمَامِ الْمُنْتَظَرِ الْمَرْضِی [الْمُرْتَضَی]وَ ابْنِ الْأَئِمَّةِ الطَّاهِرِینَ
the proof (al-Hujjah), the riser (al-Qa’im), the well-guided (al-Mahdi), the leader, the awaited, the pleased (with his Lord),the descendant of the pure Imams, the successor,
الْوَصِی ابْنِ الْأَوْصِیاءِ الْمَرْضِیینَ الْهَادِی الْمَعْصُومِ ابْنِ الْأَئِمَّةِ الْهُدَاةِ الْمَعْصُومِینَ
the son of the pleased successors (of the Prophet),the infallible guide, and the son of the Imams, the infallible guides.
السَّلامُ عَلَیکَ یا مُعِزَّ الْمُؤْمِنِینَ الْمُسْتَضْعَفِینَ
Peace be upon you, O grantor of dignity to the oppressed and faithful ones.
السَّلامُ عَلَیکَ یا مُذِلَّ الْکَافِرِینَ الْمُتَکَبِّرِینَ الظَّالِمِینَ
Peace be upon you, O humiliator of the arrogant, oppressors, and unbelievers.
السَّلامُ عَلَیکَ یا مَوْلای یا صَاحِبَ الزَّمَانِ
Peace be upon you, O my master, the Lord of the Age.
السَّلامُ عَلَیکَ یا ابْنَ رَسُولِ اللهِ
Peace be upon you, O son of Allah’s Messenger.
السَّلامُ عَلَیکَ یا ابْنَ أَمِیرِ الْمُؤْمِنِینَ
Peace be upon you, O son of the Commander of the Faithful (Imam Ali).
السَّلامُ عَلَیکَ یا ابْنَ فَاطِمَةَ الزَّهْرَاءِ سَیدَةِ نِسَاءِ الْعَالَمِینَ
Peace be upon you, O son of Fatima al-Zahra, the greatest lady of the worlds.
السَّلامُ عَلَیکَ یا ابْنَ الْأَئِمَّةِ الْحُجَجِ الْمَعْصُومِینَ وَ الْإِمَامِ عَلَى الْخَلْقِ أَجْمَعِینَ
Peace be upon you, O son of the Imams—the infallible proofs (of Allah) and the leader of all creation.
السَّلامُ عَلَیکَ یا مَوْلای سَلامَ مُخْلِصٍ لَکَ فِی الْوِلایةِ
Peace be upon you, O my master, the salutation of one who is sincere in his friendship with you.
أَشْهَدُ أَنَّکَ الْإِمَامُ الْمَهْدِی قَوْلاً وَ فِعْلاً
I bear witness that you are the well-guided Imam in word and deed;
وَ أَنْتَ الَّذِی تَمْلَأُ الْأَرْضَ قِسْطاً وَ عَدْلا بَعْدَ مَا مُلِئَتْ ظُلْماً وَ جَوْراً
it is you who will fill the earth with justice and equity after it has been filled with tyranny and oppression.
فَعَجَّلَ اللهُ فَرَجَکَ وَ سَهَّلَ مَخْرَجَکَ
So may Allah hasten your reappearance, make your advent easy,
وَ قَرَّبَ زَمَانَکَ وَ کَثَّرَ أَنْصَارَکَ وَ أَعْوَانَکَ وَ أَنْجَزَ لَکَ مَا وَعَدَکَ
make nearer the time of your coming, increase the number of your supporters and helpers, and fulfill for you all that which He has promised to you.
«فَهُوَ أَصْدَقُ الْقَائِلِینَ «وَ نُرِیدُ أَنْ نَمُنَّ عَلَى الَّذِینَ اسْتُضْعِفُوا فِی الْأَرْضِ وَ نَجْعَلَهُمْ أَئِمَّةً وَ نَجْعَلَهُمُ الْوَارِثِینَ
He is surely the Most Truthful of all speakers (and He has said): “And We willed to show favor to those who were oppressed on earth, and to make them leaders, and to make them the heirs.” (Quran, 28:5)
یا مَوْلای یا صَاحِبَ الزَّمَانِ یا ابْنَ رَسُولِ اللهِ
O my master! O Lord of the Age! O son of Allah’s Messenger!
…حَاجَتِی کَذَا وَ کَذَا
My request is for your swift reappearance.
فَاشْفَعْ لِی فِی نَجَاحِهَا
So intercede for me for its fulfillment,
فَقَدْ تَوَجَّهْتُ إِلَیکَ بِحَاجَتِی
for I am turning my face towards you for my need
لِعِلْمِی أَنَّ لَکَ عِنْدَ اللهِ شَفَاعَةً مَقْبُولَةً وَ مَقَاماً مَحْمُوداً
since I know for sure that you have with Allah an accepted intercession and a praiseworthy station.
فَبِحَقِّ مَنِ اخْتَصَّکُمْ بِأَمْرِهِ وَ ارْتَضَاکُمْ لِسِرِّهِ
So I ask you in the name of Him Who gave you (i.e. the Ahl al-Bayt) exclusively the right of representing Him, and admitted you all to keep His secrets,
وَ بِالشَّأْنِ الَّذِی لَکُمْ عِنْدَ اللهِ بَینَکُمْ وَ بَینَهُ
and I ask you for the status and rank which you have with Allah and it is only between you and Him,
سَلِ اللهَ تَعَالَى فِی نُجْحِ طَلِبَتِی وَ إِجَابَةِ دَعْوَتِی وَ کَشْفِ کُرْبَتِی
to ask Allah, the Exalted, to make my request successful, and answer my prayer, and to relieve my grief.
You may then ask for whatever you want, and it will be settled for you, by Allah’s permission.
